Antidepressant action of atypical antipsychotics: focus on ziprasidone monotherapy, with a few twists in the tale.

چکیده
Clinical Problem Mr J is a 44-year-old man who experienced a manic episode 8 years earlier. He discontinued sodium valproate maintenance therapy, because of weight gain, after about 1 year of medication use. He is presently experiencing his first major depressive episode, which is moderately severe in intensity. Mr J is reluctant to accept quetiapine because of the risks of sedation and weight gain associated with the drug. Ziprasidone is less likely to result in sedation and weight gain than quetiapine. Might Mr J respond to monotherapy with ziprasidone?

Mood stabilizers and atypical antipsychotics: bimodal treatments for bipolar disorder.
Treatment options for bipolar disorder have rapidly expanded over the last decade, but providing optimal management remains an elusive goal. The authors reviewed the literature on the efficacy of agents with the best clinical evidence supporting their use in bipolar disorder, including the mood stabilizers lithium, valproate, lamotrigine, and carbamazepine, as well as the atypical antipsychotic...
متن کاملAtypical antipsychotic drugs and diabetes mellitus in a large outpatient population: a retrospective cohort study.
PURPOSE Previous research has suggested an association between use of atypical antipsychotics and onset of diabetes mellitus. We sought to compare the incidence of new onset diabetes among patients receiving atypical antipsychotics, traditional antipsychotics or antidepressants. METHODS Retrospective cohort study of outpatients with claims for atypical antipsychotics (n = 10 265) compared to ...
متن کاملComparison of health-related quality of life among patients using atypical antipsychotics for treatment of depression: results from the National Health and Wellness Survey
BACKGROUND Use of atypical antipsychotics (AA) in combination with an antidepressant is recommended as an augmentation strategy for patients with depression. However, there is a paucity of data comparing aripiprazole and other AAs in terms of patient reported outcomes. Therefore, the objective of this study was to examine the levels of HRQoL and health utility scores in patients with depression...
متن کاملA 13-week, randomized double-blind, placebo-controlled, cross-over trial of ziprasidone in bipolar spectrum disorder.
OBJECTIVE Features of bipolarity in a major depressive disorder sample were used to define a "bipolar spectrum disorder" population for treatment with a neuroleptic agent, ziprasidone. METHODS Forty-nine acutely depressed patients were randomized to ziprasidone-washout-placebo or placebo-washout-ziprasidone in this double-blind, prospective, 13-week crossover trial. All patients met the Diagn...
متن کاملRegulation of mouse brain glycogen synthase kinase-3 by atypical antipsychotics.
Glycogen synthase kinase-3 (GSK3) has been recognized as an important enzyme that modulates many aspects of neuronal function. Accumulating evidence implicates abnormal activity of GSK3 in mood disorders and schizophrenia, and GSK3 is a potential protein kinase target for psychotropics used in these disorders. We previously reported that serotonin, a major neurotransmitter involved in mood diso...
